Access Fee:
$100 per month per family provides:

• 24/7 direct access to physicians via cell phone and secure E mail
• access to all offered services (see ”Services “ tab)

You may be able to use Health Savings Account (HSA) and Flex Savings Accounts for your access fee. Please check with your Account Administrator.

Professional fees:
Physician fees for visits range from $100 – 250 per home visit. Hospital charges may differ.

Many of our services and visits may be partially reimbursable by your insurance (depending on your coverage). You will receive an invoice that you may submit to your insurance company for our billable services. Please check with your insurance provider for reimbursement rate for out of network providers.
